  • Affidavit regarding Income/Sales Tax for not having any arrears of this Department
  • Copy of Food and Drugs Administration of Department Licence (Form C) (s)
  • No Objection Certificate of Owner of premises for sale of Liquor
  • Proof of Ownership of proposed licence premises
  • Tax Payment receipt of Municipal Corporation or other administration of Proposed Licence premises
